APPOINTMENTS

When you make an appointment in our office, we set aside a specific period of time
(routinely 30-90 minutes) for you.  If you are late, we do not extend your session
beyond the time reserved for you, because it would infringe on the next patient’s
​appointment. You will be expected to pay the full fee.  

Please make sure all minors are accompanied by a legal guardian.

Please do not leave minors in the waiting area without supervision.

Our office phone number is (678) 500-8510.  If you call during or after office hours and no one is available to answer your call, you will hear a message telling you how to leave a message or go to the ER.  
To discuss medical concerns, please do call. We welcome emails for non-urgent, administrative communication.
​Please note that the confidentiality of your email cannot be guaranteed (not HIPAA compliant).


EMERGENCIES

After hours, the phone system is forwarded to a “night message,” which gives
directions on how to leave a message.  
In case of an emergency, please call 911 or go to the nearest ER.  


Insurance:

We do not accept any Insurance.

We are considered ​as "out-of-network" provider for all insurance companies. 

If you would like to see Dr. Rajouria-Malla as an 'out of network' provider, a receipt in the form of a 'superbill' will be provided which you can submit to your insurance company for reimbursement.
You are responsible for filing your own insurance. We highly recommended that you contact your insurance company ahead of your visit to inquire about out-of-network benefits and costs. Insurance companies may reimburse only a portion of the visit. Payment for services is expected in full and at the time of your appointment. 



Payment:

Payment for services is expected in full and at the time of your appointment.
Personal checks are not accepted on the first visit/initial encounter.

Return check fee penalty is $35.

Cash and most major credit cards are accepted.

Any outstanding balance have to be paid before patient can be scheduled for follow-up.

Cancellation and No-Show Policy: 
If you are unable to keep your scheduled appointment, we request that you please call 48 hours in advance so that someone else can get the time slot. We do not overbook appointments and thus are not able to offer that time to anyone else who needs it. 

If you do not cancel your appointment at least 48 hours in advance or fail to show up
​for your appointment, you will be responsible for the full cost of the appointment.

Repeat no-shows will result in dismissal from the practice.




Paperwork Policy: 
Please note we do not accept "workers' comp" and we do not fill disability forms during initial visits. Disability forms will only be considered for long term established patients. We will not accept patients requiring evaluations for legal matters. 


If you need any paperwork filled outside of your office visit (such as for school, employment, insurance, etc) the practice may charge you a reasonable fee ($50 or more) depending upon complexities. There will be no additional charge for forms filled during an appointment, but please note that doing so will take up your treatment time as well. Please fill the HIPAA release from along with the credit card authorization from and fax it back to us. Please note that it may take upto a week to process these requests. 


Prescription refills outside of an appointment may result in a $25 charge. Prescriptions are usually calculated to be sufficient to last until your next appointment.



Emails & Phone calls:

Emails, texts/phones, computers and faxes are not guaranteed to be 100% private. Although all staff at Family Psychiatry of Georgia, LLC are required to have password protections on their computers and cell phones, it is important to realize that computers, email and cell phone communication can compromise the privacy and confidentiality of private medical infomation. Such communications are prohibited by the HIPPA act.

Please limit emails to changes in appointment times, scheduling of appointments or other brief non-medical exchanges only.

Please do not communicate confidential & private information via regular emails. You are required to log on the the patient portal for secure communications. This is clearly explained in the privacy forms & we shall not take responsibility for data breach from any emails you send to us via unsecure emails (such as yahoo/gmail/hotmail...etc).


If email or phone communication outside of sessions requires more than 10 minutes to respond to, you may be charged for professional services rendered in 15 minute increments.
